Instructions:  To reserve TLC Church Facility's you much first complete the following information:
(Use to request specific reservations of facilities and to schedule events at TLC)
Request must be received and approved 2 weeks prior to your event.

This form must be used by Outside Organizations & TLC Ministry Leaders who need scheduling approval.

The space and time you have requested will be reserved if openings are available. Please view  TLC Church Facility Usage Agreement for pricing and policy information.

After confirmation that there are no other previous reservations for that space and time you will be given approval and confirmation that will e-mailed to you as well as all TLC Ministry Leaders who's services/space you have requested. 

If there is a conflict with another ministry reservations you will be given information to help you with rescheduling.  To help you with scheduling dates, you can check the calendar of events on this website.

Your receipt of a copy of this form (approved by Pastor & Communications Director) will be confirmation of your reservation. 
 
TLC Church Facility Usage Agreement must be signed upon receiving your confirmation. 

Signature of the applicant indicates acceptance of the church facility's use procedures, rules and checklist guidelines as well as your agreement to comply with all policies.  Full payment for scheduled events must be made 2 weeks prior to event date.

Re-fundable Cleaning Deposit of $100 must be received on the day of your event in order to receive keys to facility.  Cancellation of event must be made one week prior to event date for refund and to allow other events to be planned in your event's place.
